STEP 2:  Download the Student Forms.  The three student forms (Registration & Terms of Agreement, Medical and Health Information Form, and Acknowledgement of Risk) are available in a 6-page editable PDF file.  You may print out the blank forms and fill them in, or use your computer to fill them in and then print them out and sign them.

STEP 3:  Please SIGN and FAX all three forms (6 pages) for each student to 970-728-5312 as soon as possible to secure your registration.
